Founded in 1952, VNA & Blue Water Hospice, is a non-profit home health care and hospice organization providing high-quality, cost-effective care to patients and families coping with acute, chronic, and terminal health care issues. We provide care for patients in need of short-term, long-term, and/or end-of-life care who reside in St. Clair, southern Sanilac, eastern Lapeer, and northern Macomb counties. Our team of physicians, nurses, aides, volunteers, social workers, chaplains, and therapists are dedicated and sincerely want to understand the needs of the patient before decisions are made. We inform and educate the patients and families under our care to ensure all options are understood before developing and individual care plan. Your care is our first priority. Most insurances accepted, contact us for detailed information. CHAP Accredited.
Products and Services
In-Home Health Care services: cardiac care, diabetic care, Infusion therapy, wound care, home health aides, physical therapy, occupational therapy, speech therapy, medical social work, registered dietician, palliative care. Divisions of care include: Home Health Care (skilled in-home care), Private Duty (personal, respite care, and medication set-up), In-Home Hospice Care (skilled hospice care), Blue Water Hospice Home (in-patient home-like facility).
